$15.00 co-pay for Doctor's
office visits.

Prescription Card with a
$10.00 co-pay at pharmacy. Coverage is provided
for employee, employee’s spouse and unmarried children up to age 19 (up to age
23 if certified as full-time students).

Opt Out Program (Eligible
employees who can demonstrate they are covered by another health care plan can
opt out of the County health care plan and receive $1,500 at end of full year
in which you were not enrolled in the plan.)
2. Life Insurance
- Fully paid by County while a regular
full-time employee.
- Face value $25,000.
- Coverage
begins first day of any month after sixty (60) days of service as a regular
full-time employee.
3. Retirement
Plan - All employees must and
elected officials may become members if expected to work at least 1,000 hours
per year.

Eligible employees are
required to contribute 5% of income and may elect to contribute an additional
10% of salary.

Vesting occurs after 8 years
of service to the County.
4. Holidays - The Court and
the County observe the following eleven (11) holidays:

full-time employees are eligible for paid time-off for these holidays.
5. Vacations - Vacation time is earned during a calendar year for use
during the following calendar year for all full-time employees who are
compensated for a minimum of 175 days.
Vacation leave is credited according to the schedule below for periods
of service as a regular full-time employee.
A year of service is defined as any calendar year in which the County
compensates an employee for a minimum of 175 days. A month is defined as any calendar month in which an employee is
compensated for at least 16 days.

6. Personal

7. Sick
Days - Regular full-time
employees are credited with sick leave at the end of each month of continuous
service (in which they are compensated for at least sixteen (16) days) up to a
total of ninety (90) days according to the schedule below. A month is defined as any calendar month in
which an employee is compensated for at least sixteen (16) days.

8. Bereavement
Pay - If an employee is a regular full-time employee of the
Court who has completed six (6) months of employment, he/she will be entitled
to paid leave as follows:
1. Following the death of a member of an
employee's immediate family, the Court will approve up to a maximum of four (4)
days leave of absence with pay. For
this purpose, "immediate family" is defined as including only an
employee's spouse, child, mother, father, stepmother, stepfather, and stepchild.
2. Following the death of any other member
of an employee's family, up to a 3-day leave of absence will be approved. For this purpose, "any other member of
a family" is defined as including only an employee's brother, sister,
stepbrother, stepsister, grandparent, great-grandparent, grandchild,
mother-in-law, father-in-law, daughter-in-law and son-in-law.
3. Following the death of any other
relative, an employee will be granted a leave of absence of up to one (1)
working day to attend the funeral. For
this purpose, "any other relative" is defined as including only an
employee's brother-in-law, (spouse's brother or employee's sister's husband),
sister-in-law (spouse's sister or employee's brother's wife).
9. Non-Paid
Leave - The Court provides
non-paid leave for various lengths of time for the following purposes. They are:
- Medical Leave **
- Family Leave **
- Military
Leave (15 days pay in accordance with applicable State and Federal Law)
- Educational Leave
- Personal Leave
The
Court reserves the right of approval for all non-paid leaves of absence.

Pursuant to the Federal Family and Medical Leave Act (up to 12 weeks leave
including pay for benefit time).
10.
Parking -
The County provides free parking for Court employees.
11.
Prescription Card - Mail Order (CFI) - All employees and their covered dependents can have
prescriptions for maintenance medication used in the treatment of chronic
conditions filled through mail order. A
three-month supply of the medication is mailed to the patient at one time. Prescriptions are filled with generic drugs
unless stipulated by the physician otherwise.
Brand names carry a co-pay of $10.00.

Vision Care Plan - Coverage is provided for employee, employee’s spouse and unmarried
children up to age 19 (up to age 23 if certified as full-time students). The Fund’s NVA (National Vision
Administrators) vision care plan is an examination and refraction program
designed to provide quality services to Fund members and dependents at an
affordable cost to contributing employers.
Prescription and vision benefits are an extension of medical coverage.
13. Delta Dental Program - Coverage is provided for the employee only. Coverage for spouses and dependents may be
obtained on a pretax basis. If you go
to a Pennsylvania Delta participating dentist, you can obtain covered
procedures for the following service categories at no charge based on the
"usual, customary, and reasonable" (UCR) maximum levels:
Diagnostic -(exam and x-ray)
Preventive -(teeth cleaning) Fluoride to age 19 and
sealant to age14.
Subject to an annual per
person deductible of $25.00, the following dental procedures are covered based
on the "usual, customary, and reasonable" (UCR) maximum levels:

14.
Preventive Care - Employees enrolled are given a preventive care account for each eligible
family member enrolled for coverage. This
is a pool of $250.00 per person that may be used for preventive care only.
This preventive pool is a "use it or lose it" benefit and
cannot be carried over to a new plan year.
15.
County of Carbon Flexible Benefits Plan - (through AFLAC - American Family Life Assurance Company of Columbus, Georgia)
- Voluntary supplemental health benefit
programs through payroll deductions.
- Purchased with pre-tax or after-tax
dollars.

and Hospital Intensive Care Protection.
